Hello, and thank you for asking.

1. Either. Of the choice, I'd probably use took.
2. Thank you for writing to me about this. I've just had another look at my messages to that other user, and I got it right the first time, and then mis-corrected it, which makes me look a double idiot. I'll have to go back and grovel, once new year's out of the way... In standard British English, spoken or written, you give the score of the winning team first. My team lost 3–0, won 3–0, were beaten 3–0, a 3–0 win, a 3–0 loss, Real Betis lost 4–0 at Barcelona, Real Betis lost 10–0 at home to Barcelona. It may be changing, with the normal evolution of language being accelerated by internet shorthand and by a lot of stuff on the internet being written by non-native speakers. It's not grammatically wrong, but it does sound unnatural.
3. Matter of taste. One reference at the end is enough, but if repeating the reference makes it clearer that that source verifies each part of the paragraph, so be it.
